The Perur Aadheenam has the legacy of being part of the Kailaya Meikandar Guru Parambarai, which Saint Shanthalinga Adigalaar established in the early 17th century. It has been the source of learning Tamil and Saivisim from great masters for the past 500 years. St. Ramaswamy Adigalar again revamped the Aadheenam in the early 1900s and helped society by establishing educational institutions to understand the great Tamil Saivite culture.
For the past many years, The Perur Aadheenam has been at the forefront of various community development projects, focusing on education, social welfare, and healthcare initiatives.

Under the benevolent guidance of the Adigalaars, numerous medical initiatives have been undertaken to provide relief and care to the less fortunate.Guru Mahasannidanam Thavathiru Santhalinga Ramasamy Adigalaar carried out many medical works and helped the poor people make their health their prosperous wealth. The Adigalaars thought that ordinary people should be free from diseases to excel in their lives.
In commemoration of the 80th anniversary of Adigalaar, a free Primary Health Center was established in Veiyampalayam. On the 90th anniversary of Perur Adigalaar, the Seervalarser Santhalinga Ramasamy Adikar Foundation was initiated to build a multi-specialty hospital to serve the underprivileged.

The foundation stone for the 150-bed multi-specialty hospital was laid near Muthalipalayam, the birthplace of Thavathiru Shanthalinga Ramasamy Adigalaar. The hospital, spanning 5 acres with three floors, is in the final stages of its first phase. It will include general medical consultations, maternal and child welfare services, and gradually expand to offer specialized healthcare facilities.
